Description Job Purpose: Ensuring seamless patient care by coordinating medical services and addressing patient needs.

Key Responsibilities And Duties

  • Actively engage with clients to promote wellness and preventive care. This involves educating them on the importance of regular check-ups, screenings, vaccinations, and maintaining a healthy lifestyle.
  • Collaborates with healthcare providers to develop personalized care plans that prioritize wellness and prevention for each client.
  • Conducts health assessments and screenings to identify potential health risks and recommend preventive measures.
  • Coordinates screenings for conditions such as hypertension, diabetes, cancer, or infectious diseases based on client or population needs.
  • Provides resources and information on wellness-related topics, wellness program, and services.
  • Coordinates referrals to specialists, preventive services, based on client needs.
  • Ensures seamless communication between healthcare providers to optimize client care and follow-up.
  • Serves as a central point of contact between clients, healthcare providers, and other healthcare professionals.
  • Facilitates effective communication, ensure proper documentation, and support smooth transitions of care.
  • Regularly monitor client progress in following preventive care plans, including adherence to recommended screenings and interventions.
  • Follows up with clients to address any barriers, provide support, and reinforce the importance of preventive care.
  • Builds relationships with community partners to expand access to preventive care resources and programs.
  • Maintains accurate and comprehensive records of client interactions, care plans, and outcomes.
  • Prepares reports and documentation as required by healthcare regulations and to gauge the effectiveness of wellness and preventative services.
  • Be flexible to work between different sites.
  • Performs other duties as assigned to support Wellness Aim & Objective.

Requirements Skills and Abilities:

  • Have a strong understanding of the principles and practices of preventative care, including screenings, vaccinations, lifestyle modifications, and health promotion.
  • Stays updated on current guidelines and best practices related to wellness and preventative services.
  • Demonstrates excellent communication and interpersonal skills to engage with clients, healthcare providers, and other stakeholders.
  • Be able to explain complex medical concepts in a clear and concise manner and build rapport with a diverse range of clients.
  • Possesses experience in care planning and coordination, including assessing client needs, developing clientized care plans, and coordinating services across multiple healthcare providers.
  • Understands the importance of continuity of care and have the ability to navigate through healthcare systems.
  • Have the ability to motivate and educate clients on the importance of preventive care.
  • Exhibits strong organizational and time-management skills to effectively prioritize tasks, manage caseloads, and ensure timely follow-up with clients and healthcare providers.
  • Have the ability to handle multiple responsibilities and work within established protocols and guidelines.
  • Possesses the capacity to gather, analyze, and interpret data related to wellness and preventative services.
  • Uses data to identify trends, evaluate program effectiveness, and make informed decisions to enhance care delivery and outcomes.
  • Be culturally sensitive and aware of the diverse backgrounds and healthcare needs of clients.
  • Understands the impact of cultural factors on healthcare decision-making and be able to provide equitable and client-centered care.
  • Be proficient in using electronic health records, care coordination software, and other technology platforms commonly used in healthcare settings.
  • Have a good understanding of health information privacy.
  • Works well within interdisciplinary teams, collaborating effectively with healthcare providers, community organizations, and other stakeholders to coordinate care and promote wellness.
  • Possesses strong teamwork skills to foster a culture of cooperation and shared goals.

Experience Proven work experience of at least three years as a clinical coordinator or similar role in a healthcare organization.

Education

  • Possess a bachelor's degree in a healthcare-related field such as nursing, public health, health education, or healthcare administration.
  • Additional certifications or training in care coordination, client engagement, or wellness programs would be advantageous.

Language Excellent command of oral and written English and prefer Arabic.

Licenses / Certifications Possess valid registration and classification from the Saudi Commission for Health Specialties.

Founded in Jeddah, Saudi Arabia, in 1978 by Dr. Soliman Fakeeh, the Dr. Soliman Fakeeh Hospital (DSFH) has been a true leader in the field, whose pioneering spirit and visionary resolve has - for over three decades - advanced by leaps the standards of healthcare delivery in the Kingdom and in the region.
