Care Planning: Develop and implement care or discharge plans in coordination with doctors, nurses, and other healthcare staff.

Counseling & Emotional Support: Provide guidance to patients and families dealing with stress, trauma, chronic illness, or end-of-life situations.

Discharge Coordination: Arrange post-hospital care such as rehabilitation, home healthcare, or placement in long-term care facilities.

Resource Connection: Link patients with community resources, financial aid programs, housing services, or support groups.

Advocacy: Ensure patients’ rights are respected and help them navigate healthcare systems and insurance processes.

Crisis Intervention: Assist in emergency situations like abuse cases, mental health crises, or sudden illness.
