DHA General Practitioner -HomeCare

A General Practitioner (GP) for Homecare is responsible for delivering high-quality, comprehensive medical care directly to patients in their residential settings. This role focuses on chronic disease management, geriatric care, palliative care, and post-hospitalization recovery for patients who face mobility challenges or prefer personalized care at home.

Job Summary:-
The Homecare GP conducts comprehensive medical assessments, designs tailored treatment plans, coordinates multidisciplinary care teams, and manages acute and chronic conditions outside of a traditional hospital or clinic setting.

Key Responsibilities Clinical Assessments & In-Home Care

• Conduct home visits: Perform comprehensive physical examinations and medical evaluations in the patient's home environment.
• Manage chronic conditions: Oversee long-term care plans for hypertension, diabetes, COPD, heart failure, and neurodegenerative disorders. [1]
• Prescribe medications: Manage, reconcile, and optimize complex medication regimens, keeping a close watch for adverse drug interactions.
• Perform minor procedures: Complete essential bedside interventions such as wound care management, catheter changes, and inserting nasogastric tubes.

Multidisciplinary Care Coordination

• Lead care teams: Collaborate daily with homecare nurses, physical therapists, occupational therapists, and speech pathologists.
• Coordinate specialist referrals: Connect patients with secondary or tertiary specialists when advanced clinical diagnostics or interventions are required.
• Manage hospital transitions: Facilitate smooth discharges by reviewing hospital summaries and updating home health treatment plans. [1]

Family Counseling & Documentation

• Support family caregivers: Educate family members on disease progression, daily care techniques, and emergency warning signs.
• Facilitate palliative care: Guide sensitive discussions regarding end-of-life care, pain optimization, and advanced care directives.
• Maintain digital records: Complete meticulous, real-time documentation of clinical findings and prescriptions in the electronic medical records (EMR) system.

Pay: AED8,000.00 - AED10,000.00 per month
