About the Role

The nurse coordinator is a registered nurse who functions in the role of an expert nurse clinician and resource in the area of specialty. The nurse coordinator is responsible for overseeing the clinical care and nursing activity in a specific area and for contributing to the provision of safe patient and family focused care. This role will support the running of specific clinical programs and be involved in the improvement and expansion of these programs.

Responsibilities

  • Provides nursing assessments, plan of care and evaluation of specific patient population and provides follow up care as necessary
  • Coordinates and documents patient care by utilizing the nursing EPIC access and templates. Documents all relevant information correctly, appropriately and in a timely manner
  • Assists in coordinating care of patients in their journey in the outpatient clinic, hospital stay, discharge process, and post discharge recovery period in collaboration with the multidisciplinary team members
  • Follows, monitors and reports any potential delays related to patients care and helps in ensuring timely completion of service which includes any scheduled procedures and other studies
  • Develops and implements policies, procedures and protocols related to clinical speciality in collaboration with the multidisciplinary team incorporating evidence-based practice. Evaluates and modifies as required
  • Functions as a specific service liaison nurse while working closely with the multidisciplinary team to ensure timely and frequent communication among all caregivers and patients and their families
  • Functions as a specialist nurse to provide support and education to patients and their families in the specific specialty areas and to nursing staff in conjunctions with the Nursing education Department
  • Participates in meetings that foster multidisciplinary team collaboration with the patient and their family as deemed necessary, this includes multidisciplinary meetings, discharge huddles and other meetings
  • Demonstrates collaborative multidisciplinary partnerships working towards achievements of constant improvement in quality patient care
  • Contributes to the achievement of clinical quality outcome indicators (CAUTI, HAPI, CLABSI, Falls, and others)
  • Contributes to the organizational strategic priorities (including patient experience and others)
  • Maintains records and data collection in area of responsibility. If required, develops written statistical and graphical reports indicating development and advancement towards program goals and objectives
  • Participates in research and quality improvement initiatives to support better patient outcomes
  • Be involved in the internal auditing of the service performance to ensure compliance with JCIA and DOH standards.
  • Communicates with HEART with patients and their families and the other multidisciplinary team members
  • Be accountable for establishing, maintaining and evaluating one’s own actions and lifelong learning needs
  • Performs other duties as assigned
  • Responsible for completing charge capture education and in-service training for their Institute within CCAD. When charges are not captured automatically the nurse will complete within Epic in real time, Perioperative Services within 24 business hours

Qualifications

  • Minimum of a Bachelor’s degree in nursing
  • Master’s Degree in Nursing is preferred
  • Post graduate/Certification in relevant nursing specialty is preferred

Required Skills

  • Ability to speak and write in English is required
  • Strong oral and written communication skills
  • Strong interpersonal skills
  • Ability to effectively manage multiple responsibilities simultaneously
  • Minimum of five (5) years’ experience in the specialty practice
  • Valid and current national license is required
  • BLS required
