Home Care Coordinator (Immediate Joiner)
Job Summary
The Home Care Coordinator provides administrative and operational support by coordinating home healthcare services, managing patient schedules, ensuring seamless communication between patients, caregivers, nurses, and internal departments, and maintaining accurate documentation. This role is essential in delivering high-quality home care services while ensuring efficient resource allocation, regulatory compliance, and exceptional patient satisfaction.
Key ResponsibilitiesCare Coordination
- Coordinate daily home care services by scheduling nurses, caregivers, therapists, and other healthcare professionals.
- Match healthcare staff with patients based on care requirements, location, availability, and clinical competency.
- Monitor daily schedules and adjust assignments to accommodate emergencies, cancellations, or service changes.
- Ensure continuity of care by coordinating replacement staff when required.
- Communicate care schedules and service updates to patients, families, and healthcare professionals.
Patient & Client Management
- Coordinate new patient admissions, assessments, and service initiation.
- Maintain regular communication with patients and family members to ensure satisfaction and address concerns.
- Ensure patient records, care plans, and service documentation are updated accurately.
- Assist in resolving patient complaints and escalate clinical concerns when necessary.
- Coordinate follow-up visits and ongoing care requirements.
Documentation & Compliance
- Maintain accurate patient records, scheduling documentation, and service reports.
- Ensure compliance with healthcare regulations, company policies, and accreditation standards.
- Prepare service agreements, consent forms, and required patient documentation.
- Monitor documentation completeness and coordinate with clinical teams for missing records.
- Support audits by maintaining organized documentation and records.
Administrative & Operational Support
- Prepare daily, weekly, and monthly operational reports.
- Coordinate communication between clinical teams, operations, finance, and administration.
- Monitor staff attendance, schedules, and timesheets.
- Maintain inventory requests and coordinate with procurement for required medical supplies.
- Assist in preparing operational dashboards and performance reports.
Staff Coordination
- Coordinate onboarding schedules for newly hired nurses and caregivers.
- Ensure staff licenses, certifications, and mandatory training records remain updated.
- Coordinate staff leave requests and schedule adjustments.
- Support workforce planning to ensure adequate staffing levels.
- Maintain regular communication with field staff regarding assignments and operational updates.
Cross-functional Coordination
- Work closely with Clinical Teams, Operations, HR, Finance, Procurement, and Quality departments.
- Coordinate discharge planning, service transitions, and patient referrals.
- Support implementation of new home care programs and operational initiatives.
- Ensure effective communication between departments for smooth service delivery.
Qualifications
- Bachelor's degree in Healthcare Management, Nursing, Business Administration, or a related field.
- 2–4 years of experience in home healthcare coordination, healthcare operations, patient coordination, medical administration, or healthcare scheduling.
- Experience in home healthcare services is preferred.

- Proficiency in Microsoft Office Suite (Word, Excel, PowerPoint, Outlook).
- Experience using healthcare management systems or scheduling software is an advantage.
