Position Summary
The Case Management Officer is responsible for coordinating patient care to ensure appropriate utilization of healthcare services, effective discharge planning, and optimal health outcomes. The role involves collaboration with multidisciplinary teams, insurance coordination, and patient advocacy to deliver safe, efficient, and patient-centered care.
Summary of Main Duties
- Utilization Review & Discharge Planning: Ensure patients receive medically necessary care in appropriate settings, facilitating smooth transitions from hospital to home or other facilities.
- Insurance Coordination: Provide insurers with required information for payment approvals, including prior authorizations, and work proactively to prevent claim denials.
- Comprehensive Care Coordination: Oversee the delivery of safe, timely, effective, efficient, equitable, and patient-centered care, collaborating with healthcare and insurance teams to meet patient needs.
- Case Management Oversight: Handle case assignments, develop service plans, monitor progress, and determine case closure, ensuring continuity and quality of care.
- Holistic Assessment & Care Planning: Evaluate patients' physical and mental health, preferences, and abilities to create personalized care plans.
- Collaborative Implementation: Work with patients, families, support networks, and healthcare professionals to implement and adjust care plans as necessary.
- Advocacy and Decision Support: Act as patient advocate, providing information on clinical status and treatment options to empower informed decision-making.
Responsibilities

- Collaborative Implementation: Work with patients, families, support networks, and healthcare professionals to implement and adjust care plans as necessary.
- Patient Engagement: Engage with patients to monitor progress, address concerns, and ensure satisfaction with care received.
- Documentation & Reporting: Maintain accurate records, complete necessary forms, and produce statistical reports to support case management activities.
Position Requirements/Qualifications
- Educational Background: Bachelor’s degree in Social Work, Nursing, Psychology, Healthcare Administration, or a related field.
- Professional Experience: Proven experience in case management or a related healthcare/social services role, with a minimum of 2–3 years preferred.
Certifications: Current license, certification, or registration as required by local regulations. Certifications such as Certified Case Manager (CCM) or Accredited Case Manager (ACM) are advantageous.

- Team Collaboration: Proven ability to work collaboratively within multidisciplinary teams to coordinate comprehensive care.
- Emotional Intelligence: Compassionate approach to client interactions, with the capacity to manage emotional situations professionally.
