- Provide crisis intervention and support, helping clients manage urgent situations and develop coping strategies.
- Educate clients and their families about available resources, services, and rights within the healthcare system.
- Participate in ongoing professional development to stay informed about best practices, policies, and regulations in case management.
Desired Candidate Profile
- Bachelor's degree in social work, psychology, or a related field; advanced degrees are a plus for specialized roles.
- Minimum of 4 years of relevant experience in case management or a similar client-focused role, demonstrating expertise.
- Certification in case management (CCM, CMC, or similar) is highly desirable, showcasing commitment to professional standards.
- Experience in healthcare, mental health, or social services sectors, equipping candidates with industry-specific knowledge.
- Strong organizational skills, with the ability to manage multiple cases and prioritize tasks effectively under pressure.
- Exceptional communication skills, both written and verbal, for clear interactions with clients, families, and service providers.
- Proficiency in case management software and electronic health records (EHR) systems, enhancing operational efficiency.
- Ability to work collaboratively in multidisciplinary teams, demonstrating a commitment to holistic client care and support.
- Empathy and compassion, essential for building trust with clients and understanding their unique challenges and needs.
