The Role

As our Patient Relationship Coordinator, you will be the primary point of contact between patients, their families, and our healthcare team, including caregivers, nurses, and doctors. This role demands someone who can stay calm under pressure, handle difficult conversations with professionalism, and take full ownership of the patient experience from day one.

Key Responsibilities

  • Schedule and coordinate visits for caregivers, nurses, and doctors to patient homes
  • Onboard new patients and families from initial inquiry through to care commencement
  • Handle client complaints and disputes with composure and professionalism
  • Maintain clear communication between patients, families, and field staff to keep everyone aligned on care plans
  • Keep accurate, up-to-date patient records using CRM and scheduling tools
  • Follow up regularly with patients and families to monitor satisfaction and resolve concerns early
  • Coordinate with the internal team on caregiver availability, shift changes, and last-minute adjustments

Requirements

  • Strong English communication skills — verbal and written
  • Minimum 1–2 years in a coordination, customer service, or healthcare support role
  • Experience in home healthcare is a strong plus but not required
  • Proficient with CRM software, Excel/Google Sheets, and WhatsApp-based coordination
  • Ability to stay calm and solution-oriented when handling complaints or high-pressure situations
  • Highly organized with strong attention to detail — nothing should slip through the cracks
  • A quick learner who can grasp new systems, processes, and patient needs without repeated instruction
  • Empathetic and professional when dealing with patients and families
