Nurse Team Lead
Permanent| Band 7, 36hrs/1 WTE.
The Home First Service within NHS Borders has an exciting opportunity for an experienced Registered Nurse to join our team.
Home First is focused on working differently and collaboratively with acute and community teams to support people home or within a homely setting. With a locality led community approach to reablement and broader health and social care services, the emphasis is on prevention, anticipation, and supported self-management.
The Home First service is delivered in an integrated way alongside Hospital at Home, District Nursing, Community Allied Health Professionals, Adult Social Work and Adult Social Care services to provide a holistic community response across the Borders.
The post holder will provide exemplary leadership and support to the Home First Nurse Coordinators and Health Care Support Workers to deliver short term reablement and rehabilitation to regain function and independence whilst reducing long term care requirements. The post holder holds overall responsibility for the development, implementation and evaluation of programmes of reablement and nursing care to patients in their own homes.
The post holder will be a confident decision maker, be able to work without direct supervision and directly line manage registered and unregistered staff.
We welcome applicants with an interest in Quality Improvement and Service development.
The successful candidate will be expected to support service delivery across all localities that includes Berwickshire, Central, Cheviot, Teviot & Tweeddale requiring a full UK driving licence to travel widely to meet service need.
The role requires you to be flexible to work weekdays and weekends on a rotational basis to meet the needs of the service.

However, this is only possible if the employer is a licenced Sponsor, and if the post does not fall below the current minimum salary threshold or ‘going rate’.
