Social Worker

Key Responsibilities

• Assess patients’ psychosocial, emotional, financial, and environmental needs.
• Provide counseling and emotional support to patients and their families.
• Assist patients in coping with illness, hospitalization, disability, or trauma.
• Collaborate with doctors, nurses, and other healthcare professionals to develop patient care plans.
• Coordinate discharge planning and ensure continuity of care after discharge.
• Refer patients and families to community resources, rehabilitation centers, support groups, and social services.
• Advocate for patients’ rights and access to healthcare services.
• Maintain accurate patient records and documentation in compliance with hospital policies.
• Support safeguarding and crisis intervention cases when required.
• Participate in multidisciplinary meetings and case discussions.
• Educate patients and families regarding available social and healthcare resources.
• Ensure confidentiality and ethical practice in all interactions.

Qualifications

• Bachelor’s degree in social work or related field.
• Valid professional license/classification.
• Previous experience in a hospital or healthcare setting is preferred.
• Strong communication and interpersonal skills.
• Ability to handle sensitive and stressful situations professionally.
• Knowledge of community resources and healthcare systems.
• Proficiency in documentation and report writing.
